Horrible place. They do not take care of the people. You have maybe one or two CNA’S that care if you are lucky. They leave it up to the families. If you have family here, please try to find someplace else. My mom has passed and I have bad feelings for this place!!!! Alot of families do

Absolutely awful place to put our loved ones. Used them 3 times for my parents for rehab. There were a couple CNAs and RNs that we’re good but the rest and the Social Worker is awful. If you say your concerned, worried or agree w them that discharge is not safe, they twist your words and turn you in to Adult protective services. They leave people in urine soaked beds, vomit covered gowns, make them hold their bowels to the point they vomit. The communication is awful, so is the documentation. PT do not support each one. Not recommending this facility to my worst enemy. The staff don't know anything and when I say nothing I mean nothing. Talked to an admin worker could not tell me about my Loved one but the fact that they had a bill. They don't deserve it because they don't know nothing who gets paid for not knowing anything 😶. Then on top of that they lie about the way they go about handling things and the condition that your loved one comes in. My honest opinion on how to make this place better.....It needs to be shut down or investigated... read more

As of now 1,437 people checked in here, I wonder how many checked out!? This place five stars? Yea right, You’re better off leaving your loved ones in the care of a toddler! My three year old took better care of her granny than this so called nursing/rehabilitation facility. My mother age 56, went in due to having a stroke and needing therapy. She left out malnourished, dehydrated and with a severe bed sore. The administration staff lied through their teeth, when my sister and I asked questions about her insurance, finances ... read more

not much choice in what facility you use in Jacksonville for rehabilitation. Communication here is terrible. Orders are not followed and no one will take responsibility for that. CNA's say they didn't know, nurses say they don't know why they don't know. Family members must be highly involved daily to make sure a minimum level of care is given. To be fair, there ate some really hardworking nurses and CNA's here....there are also some who spend too much time taking a break...all at the same time.
