This office is listed as a participant in the horizon book and before scheduling an appt, I called the office to confirm. After several months of visits and chares, I began noticing they were charging me based on the Horizon PPO fee schedule. I've only been going here recently but I get the upmost care. Everything is explained to me and both the doctor and assistants make sure I don't feel anxious
