I left Dr. Jalbuena after 34 months - almost 3 years since he did has never recorded my medical allergies / severe reactions to medication though it is in my medical chart when I ended up at Urgent Care on 6/28/19 and it clearly stated Discontinue with Strict ED Precautions. He wanted to give me the same medication again on 6/25/20 because Dr. Jalbuena never remembered the Severe Reaction I had when he looked at the records with me on Video.
