I called and requested that a copy of my immunization record be sent to me via fax. The person I spoke with told me that they wouldn't be able to send me the immunization record without having a signed release of information form on file. I asked her if they would be able to fax me a copy of the ROI form so I could complete it, and send it back to them. She proceeded to tell me that she couldn't do that either because they wouldn't really know it was me. This is the first time I have ever heard anything like that in my life!... read more
The ladies at the front desk are very rude and unprofessional
Horrible the nurses Brittany is awful and should not work in health care! She does not care about my pregnancy or my risks and promised me an appointment but now won’t give me one for over 2 weeks! Awful office and again BRITTANY IS THE WORST
I called pressed zero no answer. Phone rang about 20 times. This is the Health Dept? Need an emergency booster shot appointment. Press 2 for appointment but just gets a busy signal. Feel sorry for people using this department on a regular basis. Tried calling other prompts and just get recording no one can answer the phone.
