I verified that they took Humana Gold Plus and was told by Humana that there was no copay for extraction it was paid at 100% for the first three. I had been there once before very unimpressed and they charged me 5600 when I got the EOB from Humana it stated that I should only have been billed for 12.50. I took it to them they put it in there folder and that was the end of that.
