Dr Pamela Wade is REFUSING to release medical notes from a dentist appointment to both myself AND to my dentist in another state! Signed Release records were made; however, Dr Wade refuses to release to me MY RECORDS OF MY APPOINTMENT NOTES FROM THE DOCTOR …AND Dr Wade ALSO refuses to release the medical notes from my appointment to my out of state dentist when my dentist called Dr Wade’s office to specifically request Dr Wade’s medical notes from my appointment! This is against the law!
