Better off avoiding this clinic. Staff claims to require Birth Certificate for new patients for insurance purposes (because now ppl who don't have BCs can't see doctors right?) Also wanted my son's social security number- required document. I refused period. I work with DRs. It's not necessary and you can refuse don't let them tell you otherwise.
