They charged me $2,000 for 10 mins consultation. I went to Scripps emergency room as I was feeling dizzy and unwell and I just wanted to make sure it is nothing serious. I got charged $2,000 by Scripps and separate charge of another $2,000 from Pacific Emergency providers. And I have a good insurance from the employer. But somehow it didn't get applied at all. They are thieves

This is the HOSPITAL committing fraud - not your insurance company. Shame on Scripps for this. As other reviewers point out, the "Emergency" doctors are "out of network" for most insurance plans. So this means when you hit your max out of pocket, it's still not covered. This is for emergency services, and they try to sneak in exorbitant fees of thousands of dollars that insurance companies will deny because it's billed as SEPARATE.
Received a bill in April 2022 for services in August 2021. Their bill states "As a courtesy, we've filed a claim with your insurance company and expect payment from them." I called Pacific Emergency Providers to ask for a copy of the claim they submitted to my insurance and the NPI number of the doctor who treated me. They told me that they could not provide a copy of the claim as it was 'sent through their system' and that they could only give the NPI number to an insurance company. My girlfriend went to the Scripps Mercy Hospital for a reaction to a nut allergy. She was billed $4000 (after in-network insurance) by the hospital which she paid. Then was hit with a surprise $200 bill from Pacific Emergency Service in mail mail with red "FINAL NOTICE" plastered all over it, due in 14 days, otherwise it will go to collections. She has never received any other notices for this bill prior to this one. Very shady dealings by these degenerates.
