Usually reliable but fell short a couple of ways last night - I upgraded to a chopped salad as a part of my meal and after eating just over half of it, found a crumbled piece of paper in one of my bites. Let my server know and she apologized, saying she had never seen that before and would let the manager know. She also offered to bring a replacement if I wanted it.
