I have been with Oak Street S. Tempe for 2-3 years. They were fantastic when I started but then lost their primary physician and since then I have seen at least 4-5 different doctors. They can't seem to keep one steady. Their office staff changed at least twice. Their nursing staff changed multiple times. I can't even count how many drivers they have had and lost.
