I went to the office to try to talk to someone about some concerns I had about a neighbor that uses that uses this office. I was told that I should return with the neighbor so that a HIPPA waiver could be signed. I did that and was given an email address for a social worker. I contacted her via email stating I had some concerns about a neighbor and mentioned in the email that a HIPPA waiver was signed.
