Please don't use the facility. Walked in at 7:40 yes they close at 8. They asked was here told need something checked out and it was workers comp. She asked if i had authorization form i said no. She said they could not treat me, or I could use my own insurance. I said didnt want it on my insurance, she repeated nothing could be done without the form.
