Hello. I tried to fill out the forms, but I can't access them. I find it disrespectful not to inform parents that if the child isn't accompanied by both parents, they won't be seen. My son had an appointment today. My husband was out, but he can't walk, and I'm at work. I really needed my son to speak with the psychologist, and because I didn't fill out a form, they denied him the appointment.
