I visited Dr Patel for the mandatory Vaccination Report and Medical Examination needed by US Immigration Office. The receptionist/assistant gave me the reports but the form was an old version and so after i submitted it, the immigration office asked me to resubmit the form since the old version was unacceptable. The form was updated on March 30 2015, and i went for my medical examination on April 11, 2015.
