Went there for a basic checkup and was given a form asking about my gender identity and sexual orientation. This had nothing to do with my basic checkup and I told them I wasn't comfortable filling it out. They were very hostile about this and insisted I answered the questions, and even when I thought I was past that after declining the nurse came in and continued digging asking me questions even though I had been very clear none of it was relevant to my checkup.
