Walk straight away from this place. I called for a new patient appointment and they sent me a "Potential New Patient" form that requests every form of personal information about the patient applying. Flabbergasted, I called and inquired. Sure enough, they review your deeply personal information and decide whether they'll take you as a new patient, or not. Does anyone else see the problems with this?! Very unethical and I am reporting this "practice" to several authorities including local news stations.
