I have taken my 4 kids to this place for the last nearly 20 years. Every time I went, for each child, I had to fill out a long form that includes data about every person in my household. Normal things, like addresses, birthdays, SSNs, emergency contacts, phone numbers, etc. All household members on every form, every time.
