I didn't expect new patient paperwork to be 8 pages long. No email address to send paperwork back listed anywhere. I called the office several times in order to get the email to send my forms for my appointment scheduled the same day. Doctor came in the room asking questions, why I was there, asking if I was diabetic and so on. All the information was listed on the 8 pages I filled out.
