Dr. Burns removed a crown that covered an old root canal, removing enough of the tooth that it requires extraction. The tooth had been the subject of a prior endodontic evaluation that recommended that the crown not be removed and the report was in my chart. Dr. Burns either failed to review the report or ignored it's recommendation.
