I haven’t been able to reach them for months as the phone seems to have been cut off. I haven’t received any notifications. I even went by in person and it was closed. Not sure what’s going on but this is extremely unprofessional. I was waiting on them to fax my records to my new PCP and they never did even though the last time it was opened I went by in person twice and they confirmed it was being sent.

Not sure what happen this to this facility, no one answers the phone. No letter or call stating this office was closing or closed. Phone just keep ringing.
