I scheduled an appointment with this company that was in my insurance network. The appointment was scheduled 2 weeks out. On the day of the appointment I called to confirm and they said they canceled my appointment because I didn’t complete an intake form that was never sent to me. Yet I provided them all the required info they asked at the time of scheduling and said nothing about an intake form.
