All I need is a provider to sign a form for my work health insurance that shows my weight, my blood pressure, and blood sugar. For the last 3 years, getting this one simple form completed through MomDoc has been nightmare. MomDoc erodes away the cost benefit of this program by requiring 2 visits (1 to come in to request the form and 1 to get it signed), and billing you for seeing the provider and the provider signing the form. Both the main doctors are great, However the office staff makes this place not worth coming to. They are not friendly or welcoming whatsoever! They changed locations and never even bothered to leave a voicemail or text with the new location, so on the day of my appointment I showed up to a construction site! They have continually told me incorrect appointment times, which can be very frustrating when you work full time & arrange your schedule around coming to these appointments.
