This office has you sign a substance abuse agreement BEFORE they even examine you. They say they will not even consider giving you ANY medication, Whether the medications are controlled substances not, if you do not sign. I have never experienced this in my lifetime. I have only ever been made to sign this kind of agreement to get a controlled medication on a regular basis from my primary care doctor.
