Moda has been the worst for prescription coverage I have ever encountered. They would cover the brand name, then, they didn't without prior authorization (they say if this approved it only means you are able to access to the medication, no insurance costs). I tried generic, and my head hurt so bad, it wasn't worth the two-month struggle. I reached out to Moda first and they were happy to help. They said I needed my DR to fill out a DAW, which they sent to her asap and she sent back asap. I thought, ok, this is getting better... read more

I have never paid so much out of pocket for any medical plan. They seem to pay for a visit but none of the tests that go with the appointment. Just called to make sure they cover a colonoscopy (standard and covered by most when a patient hits a certain age). They told me they cover it 100%...if it is a clean colonoscopy!! If they find a polyp, they only cover 50% of it. How on earth would a patient know if they have a polyp/s or not. I was also told mammograms were covered. Moda denied my wife a mammogram and denied me a follow up screening for the Non Hodgkin's lymphoma I had in 2016. They would rather their patients get cancer.
THE WORSE INSURANCE COMPANY ON THE PLANET! You would be better off not having insurance than having to deal with the denials and problems this company has caused me in the short 3 months of having them as an insurance provider. Then their prescription plan denied a script my doctor order! Moda said they denied it because THEY wanted me to take a different medication first!
