If you have BCBS of MS, you better hope that the insurance person at MS Vision Center knows how to read your deductibles and out of pocket expenses because you will have to pay up front any way and good luck if you can get your "refund" timely! It took BCBS intervention to get mine. I had my surgery in December and it is January 23, 2015 and still not refund but I am told it will be done next week.
