I was recently referred to this Doctor due to my mother having some short term memory issues. When we arrived at the office it was dark and the door was locked. We had a 1:00 appointment and they did not come back from lunch until 1:15. Once they opened the door we sat there for another 10-15 minutes. The woman at the front desk handed me 2 very unofficial forms to fill out, only asking for my mothers name and date of birth. These forms were poorly printed and very unclear.
