SOUND LIKE SCAM. THEY WANT MONEY UPFRONT I called this doctor to find out it they take Worker's Compensation Insurance and talked to a female employee. She asked me to fill-up intake application online. When I was submitting 27 page long intake application, it asked me to provide my credit Card information. Without provide Credit Card and authorizing them to charge my credit card, it does not allow completing intake. ****Sound like scam****
