At the first intake appointment, we provided the information the paperwork required. It clearly states that if you have medicaid/medicare or no insurance then you must provide pay stubs. We have private insurance but were told we still had to provide pay stubs. I made it very clear I was not comfortable with that. They said in case the insurance lapses, they will go based on the sliding scale. (If our insurance lapses, it will be due to job loss.)
