AVOID! This clinic allowed me to "fall through the cracks" and are taking no ownership for not responding to my companies request for further details, ultimately resulting in my leaving my job. Despite 8 emails , several follow ups and a rude receptionist that assured me it was received and the doctor would get to me "when she could" they never bothered to fill it out.
