Brought my daughter there for help with ADHD, doctor felt she had it, asked for forms. We had them to Mayo the next day. Everyday we call and no one’s reviewed the forms, meanwhile my daughter suffers. We went to Marshfield Peds out of frustration, they got her in and were very nice, but now Mayo won’t fax the forms after multiple requests including a request from her Marshfield doctor.
