Did a heart cath on my mom in September of 2023 and completely missed that she needed a triple bypass resulting in a heart attack in December of 2023. My mom's regular cardiologist was never notified of the procedure done even though his name was all over her chart. Regular cardiologist asked if the heart cath was done with his eyes closed as there were 3 major blockages resulting in a triple bypass in January of 2024.
