Dr. Herman's office was phenomenal. From the front desk to the Hygienist, to Dr. Herman, everyone was extremely professional, friendly and helpful and honest. This is the first time I have not felt like a dentist was trying to rip me off with treatments I don't need!

I'm not sure what exactly you're going to do with the results of this survey. I assume the data will be compiled by the service you're using. There is potentially some very useful information that could be used in marketing campaigns to expand your practice in terms of demographic and lifestyle characteristics.
