They really wanted to schedule me but didn’t give me enough information and ended up wasting my time. I Called was told to bring 3 things: records, money and ID. I brought all of these but they needed a doctors note within a year. This would be fine but 2 other patients were there having similar issues (the staff not giving enough information). I will not be back.

My card renewal didn't go smoothly I waited 2 weeks because they didn't put in the correct email after I ask multiple times to confirm the email was correct there mistake when I called the office they said Call AZHDS not there problem they'll make mistake then be rude customer service you get what you pay for

Made an appointment due to the reviews. Got there with my paperwork filled out and waited for a little bit. The ladies there made it so easy and fast! had a great experience and once my application was submitted it took about a day or two to be approved! thanks so much! :)
