Did the staff make you feel uncomfortable when you called with questions or concerns? Absolutely! They made me feel like my questions and concerns were stupid Were you able to relax during your appointment? Yes, I was completely at ease How would you compare this provider to others in his/her field that you have visited? Among the best I have seen Did this provider use excessive foul language? Not at all! They are always respectful and courteous!

Does this provider leave you on hold for a long time when you call? Never! I don't think I've ever been put on hold here for a long time Did this provider show attention to detail? Absolutely! They are always very careful and meticulous Has this provider ever cancelled your appointment on you last minute? No, they didn't cancel on me last minute Was this provider sympathetic to your situation? Yes, they were sympathetic and caring

Did this provider pressure you to purchase any unnecessary products during your visit? Absolutely not! I was never uncomfortable Did this provider have an entertaining selection of waiting room magazines? Absolutely! They were very entertaining! Was this provider's office easy to locate? Absolutely! It couldn't have been easier! Did this provider seem up-to-date with the current advancements in their field? Yes, they were informing me of new advancements that were extremely recent

Is this provider willing to pursue advice from other providers when necessary? Yes, they were willing to ask for advice when necessary Does this provider adhere to the customer is always right policy? Yes, they listen to all complaints and make every effort to correct the situation. Did this provider's staff seem to respect him/her? Definitely! They staff spoke very highly of him/her Was this provider's office clean? Yes! I was amazed at how clean it was!

I was left alone for a while, but it wasn't too long Was this provider argumentative or easily angered? No, they were very calm and understanding Would you recommend this provider to others?
