I scheduled an appointment and the woman started taking down all my information. She wanted my SSN and would not accept just our insurance ID numbers and my husband’s date of birth (since he’s the primary). I told her we don’t give our SSN out due to identity theft. She refused me service so I’ll take my business elsewhere.
