They put the wrong information for my insurance and said my insurance was inactivated when it was not. Therefore, I had to pay out of pocket and contact my insurance company. I have Anthem BlueCrossBlueShield and it was never inactivated. When I talked to one of their insurance person, she said my group number didn't matter when I clearly had a different group number from what she had.
