After being injured on the job in front of terminal manager, dock supervisor and 2 other workers I went to Dr. that company had account with. at that time i was put on restrictions and was given physical therapy. This all started on November16th of 2022. I did all the things that the Dr. wanted went back and he told me that he wanted to take me off of my restrictions for 30 days then reevaluate my injury. Dave Bialke was recommended to me by a high school friend. I was looking for help with a work comp claim that had been repeatedly denied by SFM Insurance.
