Kulshan Pediatric Dentistry removed two of my 6 year old son's teeth after I had explicitly informed them I wanted none of my son's teeth removed. They have also, on multiple occasions, refused to provide my son's dental records. Thankfully, the Department of Health and Human Services has been very effective at persuading Kulshan to adhere to the HIPAA laws.
