Every question I had was answered thoroughly Does this provider give unbiased advice? Absolutely! I always get sound, helpful, and completely unbiased advice! Is this provider easy to reach in an emergency? Yes, they answered my call right away Did you feel safe in this provider's care? Absolutely! I knew I was being cared for by an expert!

Was it clear that this provider takes pride in the appearance of his/her office? Not really, it wasn't very clean Did this provider wear an obnoxious cologne? Yes, it was almost unbearable Does this provider leave you on hold for a long time when you call? Yes, it never fails. I'm put on hold forever when I call Are you going to visit this provider again? Not if my life depended on it Did this provider have an entertaining selection of waiting room magazines? Yes

Did this provider thoroughly explain the risks and benefits of your treatment? Yes, they made sure I had all the information I needed to make a decision Does this provider remember you by name? Sometimes, but not always Did you experience unnecessary pain during your visit? A little, but it was bearable

No, I left with more problems than I came with! Was this provider's office easy to locate? No, I couldn't even find it with my GPS Are you going to visit this provider again? Not if my life depended on it
