Did this provider seem up-to-date with the current advancements in their field? Yes, they were informing me of new advancements that were extremely recent Did this provider show attention to detail? Absolutely! They are always very careful and meticulous Did this provider ever dismiss your concerns as unimportant? Never, they go out of their way to address every concern of mine immediately

Was it clear that this provider takes pride in the appearance of his/her office? Definitely! It was one of the best looking offices I've ever seen! Did this provider ever postpone your appointment? Not at all! They always try to get me in as soon as possible Were the waiting room chairs comfortable at this provider's office? Yes

Do you feel that you could have received better service somewhere else? Definitely not; I received the best service ever! Did a staff member acknowledge or greet you when you entered? Yes, I was greeted when I walked in Did you notice a foul odor when you arrived at this provider's office? No, I didn't notice an odor Does this provider use current and safe equipment? Yes; the equipment is very advanced! Does this provider welcome questions? Absolutely! They are happy to answer all of my questions

Yes, it was handicap-accessible Were the restrooms clean at this provider's office? Yes, I could tell they were cleaned regularly
