FYI he has moved his practice to Burlington as of October 2024.
Worst Doctor I've been to. He didn't follow up with my doctor, he made me wait for an hour for my appointment, which was 17 minutes long. He sent me home with a handful of prescriptions without listening to my concerns. He didn't listen to my symptoms, didn't wait for me to finished explaining my mental health diagnoses, but then prescribed me with medication that could affect me negatively. Don't waste your time.
You guys moved location and haven't informed any patients. Health care at its finest.
Dr. Kozak is an amazing Doctor who saved my sisters life. The only reason why I'm rating his office so poorly is because the secretary refuses to book my sister another appointment. When we ask why, they say we need to find a doctor closer to the area we live in. Their tone & attitude is horrendous and we are schocked because the previous workers were always so kind. Because of my sister not being able to see Dr. Kozak for over 2 years, her mental health is declining & we cannot find another doctor good enough to help her. H... read more
