Did a staff member acknowledge or greet you when you entered? Yes, I was greeted when I walked in Did this provider rush your appointment? No, I was able to ask as many questions as I wanted Does this provider remember you by name? Every single time, even if it's been months since my last appointment!! Does this provider always put the customer first? Yes, they always put customers first

Is this provider easy to reach in an emergency? Yes, they answered my call right away Was it clear that this provider takes pride in the appearance of his/her office? Yes, the office was well organized and taken care of Does this provider leave you on hold for a long time when you call? No, they are really great about answering the phone right away Were the staff members able to answer your questions? Yes, they were able to answer my questions Did this provider prominently display their diplomas or business license? Yes

Did this provider prominently display their diplomas or business license? Yes Did this provider ever postpone your appointment? Not at all! They always try to get me in as soon as possible Did you spend a lot of time in the waiting room at this provider's office? Absolutely not! I was seen right away! Did this provider's staff seem to respect him/her? Definitely! They staff spoke very highly of him/her

They aren't known for being that good at what they do in my area, but they aren't known for being terrible either. They should probably work on improving their professional reputation. They seemed like they were using fairly modern treatment methods and procedures, but I'm not really sure. They weren't very understanding or caring, and they didn't show empathy for my situation. I could tell I wasn't their primary concern. They seemed to know what they were talking about most of the time, but I still wasn't very comfortable.

Did you have to wait long after you arrived for your appointment? I had to wait for a little while. Is this provider easy to reach in an emergency? Yes, they answered my call right away Does this provider offer flexible appointment times? Yes Does this provider maintain a professional appearance? Yes, he/she looked very professional
