Horrible experience attempting to get pre surgery vaccines. Take note that just because your insurance is listed as accepted, and you spoke to a department nurse to verify coverage, it might not be covered. Also calling to verify forms needed is not a guarantee that they will accept your forms. It was vitally important that the form be presented with my hand written name and dob on it.
