This Dr was given a good reference from a friend. I have been trying to make an appointment for over a year. NO ONE ANSWERS! Finally, someone calls me back & tells me they will send me a packed before making me an appointment. They take all my info. Its been a month since that call and I haven't received a packed or an appointment. I have called several times about packed and AGAIN NO ON ANSWERS THE PHONE. The only reason why I am constantly calling is because I trust the person who gave the ref
