Had a job done that required alignment of belts/pulleys after completion. No other persons adjusted anything after the job was completed. 3 months later we have a motor jumping and find the pulley off alignment. A month later we happen to spot check a second motor and it's pulley wasn't even tightened onto the shaft and was 3/8" off alignment.
