Does this provider always take that extra step to make you feel special? No, I don't think they appreciate my loyalty Does this provider remember you by name? No, and they don't make an effort to even try Was this provider late to your appointments? Sometimes, it was a little frustrating Did you have to wait long after you arrived for your appointment? I had to wait for a little while. Are you going to visit this provider again? Not if my life depended on it

Did this provider show attention to detail? Yes, I've never had to worry about them missing any important information Was this provider sympathetic to your situation? Yes, they were sympathetic and caring Did this provider promise services he/she couldn't provide? No, they were able to provide everything they said they would Was there annoying music playing while you waited or during your visit? No, there was no annoying music Did this provider listen to your input and concerns? Yes

Yes, they went above and beyond Did you leave the office feeling satisfied with your visit? Completely! Was this provider's office easy to locate? Yes, they always put customers first Does this provider always take that extra step to make you feel special? Yes, I always enjoy my appointments Is this provider's business conveniently located? Yes, it is very conveniently located Did this provider ever postpone your appointment? No, they never postpone my appointment

Was it clear that this provider takes pride in the appearance of his/her office? Definitely! It was one of the best looking offices I've ever seen! Were the waiting room chairs comfortable at this provider's office? Extremely comfortable; I could have fallen asleep Was it easy to make an appointment with this provider? Yes! It was effortless! Does this provider promptly return your phone calls? Definitely! They always call me back very quickly! Did this provider prominently display their diplomas or business license? Yes

No, I don't think so Was it clear that this provider takes pride in the appearance of his/her office? Yes, the office was well organized and taken care of Is this business handicap-accessible? Absolutely, even the bathrooms are handicap-accessible Did this provider thoroughly explain the risks and benefits of your treatment? Yes, they made sure I had all the information I needed to make a decision

Does this provider adhere to the customer is always right policy? Absolutely not!! They never listen to customers complaints and argue with them Were the staff members able to answer your questions? Every time I asked the staff a question, they referred me to someone else Did this provider ever dismiss your concerns as unimportant? No, they take every concern of mine seriously Does this provider ever overbook appointments here? Were you able to get answers to simple questions from this provider without scheduling an appointment? No, this provider rarely answers questions over the phone Is this provider easy to reach in an emergency? Absolutely not. I was forced to call the emergency room.
