I’m only writing this review of one star because I can’t leave a zero stars.My experience is with Wendy Kozlow. I can’t even bring myself to put DR in front of her name because I don’t think that she deserves that title. I was hospitalized a month ago because I was placed on a medication by a previous endocrinologist, Jardiance, and it put me into diabetic ketoacidosis. Awesome Dr Perez and Nurse Practitioner they have enriched my life. However the Staff responsible for following up on Prescriptions and annual certificate of Medical necessity absolutely incompetent as I've been trying with a provider now 22 days and 6 phone calls and still they can't seem to get it done. On two occasions they told my provider I was not a patient event thought I've been more than 10 years.
