Do not go to this place. After seeing Amanda Butler twice, she prescribed two medications that created the same side effects. I did miss one appointment with her and did not come back for the last appointment, as she refused to provide the short-term medical leave form that my employer requires. I contacted them twice via their website and called them two times with no response from response from Amanda.
