I have United Healthcare and my co-pay is supposed to be $0. When I had my eye exam in February he charged me $45 and since then I've been trying to get a refund. I even been on a three-way call with my insurance company and the doctor and he was supposed to resend the claim because it was processed incorrectly and as live yet he has not done so. It's been 7 months. Don't get scammed.
