If I could give this insurance zero stars I would. This is by far the worst insurance I've ever had. They claim to cover 2 well visits a year but they don't, I have just received a bill for $184.67 for my 15 minute checkup with labs. I have paid 100% of my medical bills so far this year. They pretend they are paying part of the costs but all they're doing (supposedly) is making a deal with the provider and/or pharmacy as to what the costs will be. When you find a Dr. that will take this insurance that no one has heard of. They denied my claim twice, saying it was deemed unnecessary. After being on the phone for an hour, the representative told me on the 3rd appeal that they would have their Dr. To review my case. So the people who decides that it was unnecessary wasn't a Dr., By the way I have severe drop foot , Stumble and fall 2 to 3 daily until I got a foot brace. I heard of insurance scams . It appears to me the scam is the insurance company itself!

Can't find a doctor or hospital in my area that will take this, "Insurance ". Seems kinda bad when you call every doctor in my area and none will accept it.

Fake insurance meant to appear as though the employer provides health insurance. Go to the Imagine website, only serve about 6 cities in the entire USA. Virtually no providers. No one accepts there insurance, and when they do, you get the bill anyway.
